A human stroke drug, fast-tracked by the Food and Drug Administration<\/a>\u00a0and soon entering Phase 3 clinical trials, shows intriguing signs that it might also be a safe and powerful defense against Alzheimer\u2019s and other forms of dementia.<\/p>\n

A new study published Tuesday in the\u00a0Journal of Experimental Medicine<\/a>\u00a0showed the drug,\u00a03K3A-APC<\/a>, protected mice from injury to the brain\u2019s white matter \u2014 the second leading cause of\u00a0dementia<\/a>\u00a0in humans. Such injuries occur when tiny clots block the flow of blood perfusing the brain\u2019s white matter. Over time, these mini-strokes multiply and lead to cognitive decline.<\/p>\n

\u201cThe results suggest the drug\u2019s potential for treating white matter strokes in humans, possibly including multiple white matter strokes that result in vascular dementia,\u201d said senior author\u00a0Berislav Zlokovic<\/a>, director of the\u00a0Zilkha Neurogenetic Institute<\/a>\u00a0and professor and chair of the Department of Physiology and Neuroscience at the\u00a0Keck School of Medicine of USC<\/a>.<\/p>\n

Wider impact on Alzheimer\u2019s disease<\/strong><\/p>\n

The stroke drug could potentially be used in people with widespread, diagnosed white matter injuries that have developed over time, researchers said, with the goal of slowing down progression that leads to cognitive impairment.<\/p>\n

The finding adds to the substantial list of benefits of 3K3A-APC already seen in mouse models of Alzheimer\u2019s disease. A\u00a0previous study<\/a>\u00a0in mice showed that the drug boosted the health of brain vessels, improved blood flow, reduced brain inflammation and cut down on the buildup of amyloid, the protein commonly found in the brains of people with Alzheimer\u2019s disease.<\/p>\n

The drug is a genetically modified version of a human blood protein called activated protein C, or APC. It reduces inflammation and protects both neurons and the brain vascular system from death and degeneration. Zlokovic co-discovered the protective effects of APC in the central nervous system.<\/p>\n

Fast-tracked clinical trials on stroke drug may benefit dementia treatment<\/strong><\/p>\n

As mouse-model studies have advanced, so have clinical trials in humans. In 2019, the drug appeared safe in a\u00a0Phase 2 clinical trial<\/a>\u00a0in people with strokes caused by blood clots who also received a standard of care therapy with a clot-dissolving drug or surgical removal of the clot. The drug reduced brain bleeding for humans, just as had been seen in mouse models.<\/p>\n

The Phase 3 trial for 3K3A-APC in ischemic stroke is expected to start in 2022. Additionally, a small Phase 2 trial in patients with amyotrophic lateral sclerosis started this month.<\/p>\n

\u201cOur recent and current data support development of 3K3A-APC for neurological conditions associated with cognitive dysfunction such as Alzheimer\u2019s disease and injury to the brain\u2019s white matter that results in vascular dementia,\u201d said Zlokovic.<\/p>\n

In addition to Zlokovic, other authors include co-first authors Mikko Huuskonen, Yaoming Wang and Angeliki Mari Nikolakopoulou, all of the Keck School of Medicine, and Axel Montagne of the University of Edinburgh; and Zhonghua Dai, Divna Lazic, Abhay Sagare and Zhen Zhao, also of the Keck School; Jose Fernandez of the Scripps Research Institute; and John Griffin of Scripps and the University of California, San Diego.<\/p>\n
