The\u00a0doctors of the sarcoma program at the USC Norris\u00a0Comprehensive Cancer Center\u00a0<\/a>recently\u00a0enrolled their final patient for a new clinical trial to test an immunotherapy treatment for sarcoma, a fact that has James Hu, MD, medical director of the sarcoma program, feeling a little more buoyant than normal.
\nLaunching a new clinical trial is always an\u00a0exciting moment, but for the doctors at the\u00a0sarcoma program at USC Norris, it is particularly\u00a0satisfying because the disease they treat is\u00a0rare and new treatments have been elusive.
\n\u201cThere hasn\u2019t been a new therapy for high\u00a0grade sarcoma in over 20 years,\u201d says Hu,\u00a0noting that USC Norris is the only west coast\u00a0cancer center involved in testing an immunotherapy\u00a0drug for advanced sarcoma, the family\u00a0of cancers that develop in connective tissue.
\nWith fewer than 15,000 new cases a year,\u00a0sarcoma is the least common form of cancer.\u00a0It also has the largest number of subtypes,\u00a0about 70, meaning some of the subtypes rarely\u00a0show themselves.
\nBeing small in numbers means that the\u00a0word less applies often to sarcoma. There is less funding for research on sarcoma and less\u00a0is known about its underlying causes. Its rarity\u00a0also means that many doctors, including\u00a0oncologists who treat other cancers, have little\u00a0or no experience treating sarcoma.<\/p>\n

Volume Matters<\/h3>\n

Because sarcoma is uncommon, only a\u00a0handful of academic cancer centers, including\u00a0the program at USC Norris, have a wealth\u00a0of experience in treating sarcoma patients.\u00a0The already busy practice at USC Norris is\u00a0about to get busier because they recently\u00a0teamed up with the sarcoma specialists at Hoag\u00a0Hospital in Newport Beach, a move that will\u00a0offer sarcoma patients in Orange County access\u00a0to USC Norris\u2019 services and clinical trials.
\nExperience is more important when it comes\u00a0to sarcoma because it is more difficult to treat\u00a0than other types of cancer. \u201cSarcomas are different\u00a0animals and they defy all the fundamentals\u00a0you have learned about cancer,\u201d says Hu.
\nSarcoma develops in bone, muscle, \u00a0cartilage, fat or other connective tissue in any\u00a0part of the body, one reason it is hard to\u00a0diagnose. Making a proper diagnosis requires a\u00a0multidisciplinary team, including pathologists\u00a0experienced with sarcoma to accurately\u00a0diagnose the many subtypes.
\nOnly surgeons at USC Norris do biopsies\u00a0when sarcoma is suspected because this\u00a0is also more complicated than other types of\u00a0biopsy and, when done poorly, can be costly to\u00a0patients both in dollar terms and in terms of\u00a0their prognosis.
\nTreating sarcoma typically requires some\u00a0combination of surgery, chemotherapy and\u00a0radiation. Lawrence Menendez, MD, chief of\u00a0the center for musculoskeletal oncology, says that\u00a0the combination of treatments requires perfect\u00a0timing, but also a flexibility to switch treatments\u00a0when necessary. \u201cKnowing how to do this right\u00a0only comes from experience,\u201d says Menendez.
\nFor years, the team at USC Norris has been\u00a0an early adopter of pioneering treatments for\u00a0sarcoma patients to effectively remove tumors,\u00a0minimize recovery time and maximize functionality.\u00a0The team has been at the forefront of\u00a0employing 3-D computer imaging to navigate\u00a0during surgery and utilizing leading edge technology\u00a0such as cryoablation and radiofrequency\u00a0ablation to kill tumor cells.
\nNovel techniques have been developed by\u00a0the USC Norris team to deliver high doses of\u00a0radiation to cancer cells, without the toxicity\u00a0of some types of radiation, and have developed\u00a0effective techniques to combine targeted\u00a0radiation therapy with chemotherapy. They are\u00a0currently developing new procedures to reduce\u00a0radiation treatment for sarcoma patients from\u00a0five to six weeks to about five days.<\/p>\n

Robust Research<\/h3>\n

They are also constantly pushing on the research front, both in the lab and in clinical trials. There is research underway to study genomic changes in sarcoma tumors before and after therapy. Another research project
\nis evaluating new imaging techniques to better\u00a0assess the response of patients with soft tissue\u00a0sarcoma to treatment.
\nThere are several other clinical trials\u00a0underway for specific subtypes of sarcoma,\u00a0some of which are showing promising early\u00a0results, says Menendez. The team is also\u00a0preparing to open an additional clinical trial\u00a0on another new treatment for a broad range\u00a0of tumors, including osteosarcoma and other\u00a0sarcomas, designed to attack cancer in a novel\u00a0fashion \u2014 by halting the ability of tumor cells\u00a0to grow the blood vessels they need to strike\u00a0and later spread cancer.
\nAlthough it will be some time before the team\u00a0will be able to report preliminary results from\u00a0the newest trials, such as the immunotherapy\u00a0trial, Hu says he is hopeful. \u201cIt is pretty exciting\u00a0and we are cautiously optimistic,\u201d he says.<\/p>\n\n\n\n<\/div>\n <\/div>\n\n\n
