class=\"chcs-post-content\">\n<p class=\"p1\">People experiencing homelessness face numerous obstacles in accessing care and managing their health and social needs. Street medicine care teams deliver care to people who are unsheltered within their own lived environment, helping them access critical health and social services. In California, as in other states, sustainable funding is needed to support the growth and integration of street medicine into the broader continuum of health care services.<\/p>\n<p>This toolkit, made possible through support from the California Health Care Foundation and Health Net, is designed for California-based health care providers and community-based organizations seeking to financially support existing street medicine programs, or those designing and launching new programs. It includes practical, stepwise guidance and tools to help street medicine programs maximize their use of available funding, particularly new opportunities introduced through CalAIM.
