Heart Health Learning Collaborative

Maintaining heart health throughout life can help reduce the risk of heart disease.  Although heart disease is the leading cause of death both nationally and in Los Angeles County, managing health conditions, such as hypertension, can significantly reduce its risk. Certain communities, however, face numerous challenges to managing hypertension and maintaining heart health. Unmet social needs and socioeconomic adversity can inhibit a person’s ability to prevent and manage hypertension. Communities that are disproportionately impacted by these conditions are more vulnerable to hypertension and heart disease. The Los Angeles County Heart Health Learning Collaborative (HHLC) is working to develop, implement, evaluate, and disseminate strategies designed to enable individuals to overcome barriers to managing hypertension. By utilizing rapid cycle quality improvement and peer learning, HHLC is developing practices that help bridge gaps in equity faced by the communities in Los Angeles County that are most impacted by cardiovascular disease.

Mission

The mission of the HHLC is to bring together a multiagency and multidisciplinary group that works together to develop implementation and evaluation strategies which promote hypertension control, heart health, and social wellness within the Los Angeles County communities that need it most.

Vision

The vision of the HHLC is for all Angelenos at risk for cardiovascular disease to receive high-quality, effective, and equitable care to prevent, detect, control, and manage hypertension.
