Application Information

Instructions for Medical Board of California License

Keck School of Medicine must submit the following documents to support your license application to the California Medical Board:

  1. Certification of Medical Education Form (FormMED)
  2. Official Transcript
  3. Certified copy of Diploma
  • Complete the top portion of FormMED (Certificate of Medical Education) which you’ll find in the license application packet.  Upload the form to Parchment when you order the certified copy of the diploma and official transcript. You can also email the form to medstuaf@usc.edu.
  • Order a certified copy of the diploma and official transcript online through Parchment. The fee for the transcript is $15. There is no charge for the certified copy of the diploma. Rush order delivery is available for an additional fee.

Instructions for other State Medical Board Licensure

  • Complete/sign the applicant information section on the form and upload the form to Parchment when you order the Certified Diploma, Official Transcript and/or MSPE (Dean’s Letter). You can also email the form to medstuaf@usc.edu.
  • Order a certified copy of the diploma, official transcript and/or MSPE (Dean’s Letter) online through Parchment. The fee for the official transcript and MSPE (Dean’s Letter) is $15 for each document.  There is no fee for the certified copy of the diploma. Rush order delivery is available for an additional fee.

Instructions for Fellowship Application through ERAS/MIDAS

Order an official transcript and/or MSPE (Dean’s Letter) online through Parchment. The fee for the official transcript and MSPE (Dean’s Letter) is $15 for each document. For the Destination Field, please enter your own contact information as these fields are required. In the Order Options/Special Instructions box, please state that these documents are to be uploaded to MIDUS.
