This form is for Keck School students and guests (including non-Keck School faculty and non-Keck School staff) to request reservations at the Keck School of Medicine of USC. Keck School faculty and staff should log in directly to 25Live (USC login required) to submit reservation requests (logging in will reduce data entry needs).
