Introducing a unique opportunity to transform healthcare for people experiencing unsheltered homelessness across California! This new learning initiative combines in-depth education on key issues affecting homeless patients, exploration of innovative policies, and the development of actionable plans to drive real change. By joining an EAG, you’ll gain invaluable insights, collaborate with experts, and leave equipped to make a tangible impact on care for one of our most vulnerable groups.

Enhanced Care Management:

Registration is open!

Our first EAG will focus on determining best practices for Enhanced Care Management (ECM) in street medicine! This EAG will meet every other Wednesday at 3pm beginning April 16, 2025 through June 25, 2025. All team members are welcome to join but must register (free!) to receive meeting information.
